Work Authorization

Note: all fields in the work authorization form below are required.

I hereby authorize the repair of my vehicle,

I agree that Armadillo Collision Central is not responsible for loss or damage of this vehicle and/or the articles left in the vehicle due to fire, theft or any other cause beyond its control or for any delays caused by the unavailability of parts or shipping delays. I also hereby grant permission to the company’s employees to operate the vehicle for the purpose of testing or inspection.

To secure payment in the amount of repairs, an expressed mechanic’s lien on the vehicle is acknowledged and I further agree to pay reasonable attorney’s fees and court costs in the event that legal action becomes necessary to enforce this contract. My signature constitutes and acknowledges that upon completion or authorized repairs, this motor vehicle may be subject to repossession for an expressed mechanic lien under Texas Property Code 70.001 for non-payment of any kind. I also authorize Armadillo Collision Central to endorse all insurance checks pertaining to this invoice. I acknowledge that if closer analysis reveals additional repairs are necessary I will be contacted for authorization of any additional repair charges. If new parts listed in the attached repair order are not available, Armadillo Collision Central reserves the right to repair such damaged or worn parts when possible. The difference between the part price and the labor required will be adjusted accordingly.

Terms: The total amount of the repair charges must be paid before the attached vehicle can be released for delivery. If insurance coverage pays either a portion of, or the total amount due, I acknowledge that the insurance check/draft must be obtained by myself or sent in advance by the insurance company. I also acknowledge that I must make arrangements with any lien holders or other payees to endorse the insurance check/draft prior to the release of the repaired vehicle as described. All deductibles and customer pay repairs are to be paid prior to delivery of the vehicle. Armadillo Collision Central accepts the following forms of payment:

• MasterCard
• Cash
• Money Order
• Certified Check
• Personal Check (not to exceed $1000.00)

*Please note: All personal checks will be electronically transferred through Cross Check. By paying by check, you authorize Cross Check or their agent to process the check by traditional deposit, electronically, or through a ‘preauthorized draft.’ If the check is returned for any reason, you authorize Cross Check or their agent to re-present the check one of more times by such methods, and you authorize Cross Check or their agent to collect a returned check fee up to the maximum amount as permitted by law.

For the consideration of the repairs made to my vehicle,

I grant my power of attorney to sign or endorse any checks and/or drafts made payable to me and any release thereto as settlement for my claim for damage to this vehicle.
